Payments can be set up as a recurring monthly charge by using a credit card, debit card, or ACH bank draft. They can also accept your monthly payments made by personal check, cashier’s check, or money order. If you elect to pay by personal check, cashier’s check or money order, detach and return the coupon portion from your billing statement with your payment.   • Fill in all required information.
  • Attach the completed claim form and other documents to upload.
  • Click Submit.
